By submitting this form electronically, I, the parent or legal guardian of the above-named minor, hereby give permission for my child to participate in the youth session of the Men’s Summit hosted by Remnant Bible Fellowship Church, Inc (Remnant Church). 

Consent & Acknowledgement

I understand that while every reasonable effort will be made to provide a safe environment, participation in youth activities involves inherent risks, including but not limited to physical activity and group interaction. I agree to release and hold harmless Remnant Church, its leaders, employees, volunteers, and representatives from any and all liability for injuries, losses, or damages that may occur during or as a result of my child’s participation.

Medical Authorization

In the event of a medical emergency where I cannot be reached, I authorize the event organizers and adult leaders to secure medical care for my child as deemed necessary, including transport to a medical facility. I understand I am financially responsible for any medical treatment provided.

Photo and Media Release

I give permission for Remnant Church to take photographs, videos, and digital images of my child during the event. These images may be used for church promotional purposes including social media, websites, and printed materials without compensation or further notice. I waive the right to inspect or approve the final use of these materials.
